The Part-Time FAN form has beeen updated and is on the OFR website under the Forms section at:
http://uwo.ca/pvp/facultyrelations/administration/forms-guides.html
Changes are simply a deletion of one of the Cancellation lines and correction of a typo at the bottom of the form in the Clauses.
